This presentation is designed to acquaint attendees with a unique approach to neurological rehab. The rehab approach that will be presented focuses on providing functional rehab in a patient’s natural environment, including their home and community. This type of treatment is especially effective with individuals who have challenges generalizing skills they have been learning in medical model treatment settings.
Learning objectives:
♦ Define home and community rehabilitation and differentiate it from other levels of care.
♦ Differentiate the roles of the various disciplines on the team within the home & community model.
♦ Recall common outcomes for patients completing home and community rehabilitation.
♦ Describe the difference between a functional therapy approach and a traditional therapy approach.
♦ Identify the potential cost savings of delivering rehabilitation services in the patient's own environment.
♦ Discuss the types of individuals who benefit from this model of care including diagnosis.
